Members of this team include:
-
Nurses,
who assess patient needs regularly and consult with the patient’s
physician about treatment. Nurses perform medical care
procedures such as wound care, placing and maintaining catheters
and expert
pain and
symptom management. Nurses also provide education to the
patient and family regarding end of life issues or each families
skills
needed to care for their loved one. Nurses are available
24 hours per day.

Social
workers help patients and families deal with personal, financial, emotional
and care planning needs. The Social
Worker may help identify
community resources and provide referrals, assist with paperwork
for insurance or assistance programs and assist with the
emotional side
of the dying process. They also offer bereavement services for
the family and loved ones.

Home
health aides provide personal care to the patient such as bathing, skin
care and linens changes. Aides can also
assist with
light housekeeping
and meal preparation.
Physical, Occupational and Speech therapists help maximize quality
of life and safety of patient and family. Therapists may visit
the home to evaluate need for medical equipment or to teach transfers
or
positioning of the patient. Speech therapists provide assistance
with speech as well as swallowing difficulties.

Chaplain, who
is available to help both patient and family with spiritual concerns.
They also offer bereavement counseling.

Volunteers may stay with patient to provide one-to-one companionship or
allow the family to take a break. They may provide transportation
to appointments or run errands.

Support
staff who are knowledgeable and competent to address
any questions or quickly direct to the person needed.

Respite
care can also be provided for families needing several days away
from care giving. Continuous
care may be an
option in times of crisis or imminent death.
-
Medical
equipment, supplies and some medications necessary
to ensure comfort are also provided by hospice.
-
Bereavement
services are offered at no-charge for the
family and loved ones of the patients for 13 months
as part of
hospice services.
